THE QUESTIONNAIRE
Several pages of multiple-choice questions are followed by a final page that allows you to enter contact information, report format options and all the medical details not covered elsewhere. The questionnaire can take as much as several hours to complete, depending on how many notes you enter. Each time you submit a page, an email is sent to us with your answers and notes for that page. You can fill the questionnaire out over several sessions or days if you need to. It is best to complete any page you are on and send it before taking a long break. If you do stop to take a break, close your browser or turn your computer off, none of the answers you have already submitted will be lost.

To give you an idea how far along in the questionnaire you are, look to the upper left hand of each page. This will tell you which page you are on of the total number of pages there will be. Only the main questionnaire pages are counted in this total - there may be many subpages depending on your answers. Because the number of questions will vary based upon your specific answers, we can't tell you how many questions there will be left for you to answer, but being on "Page 4 of 8" gives you somewhat of an idea.



ANSWERING THE MULTIPLE-CHOICE QUESTIONS
Select one of up to five choices.  Each question has a default answer pre-selected and this will often be the correct answer for you.  You may skip a question (leave the answer as it is) if:
  • You agree with the default answer
  • You are unable or unwilling to answer the question
  • The question does not apply to you.


ADDING FURTHER COMMENTS
For a variety of reasons you may wish to add clarifying comments, for example if you are not sure of which answer to select or you need to give us some extra information.  This can be done in the comment box at the end of each section of questions.  We use these comments not only to provide you with a better report, but also to improve the questionnaire.


SAVING AND CHANGING YOUR ANSWERS
Each time you move to the next page your answers are sent to The Analyst™ and stored in its database.  If you would like to go back and change some answers, that is not a problem. The latest answers you send will be used.

Changing your answers on pages that you have already submitted should be done during the same sitting by using your 'back' button, making your changes to the page in question and resubmitting that page. This way your old answers and notes are preserved for you to see and change. Once you close your web browser or shut your computer down, you can no longer review the answers and notes that you have already submitted.

If you can not complete the questionnaire before closing your web browser or shutting your computer down, when you return, you will be given the choice of continuing from where you left off or starting over, but your old answers will not be visible for you to review.



RECENT LAB/DIAGNOSTIC TEST RESULTS
If you have had any lab tests performed and are able to tell us the results, this can help us confirm or rule out certain conditions.  You will be given the opportunity to enter these results towards the end of the questionnaire.  If you do not have them with you now, you can make a start on the questionnaire and return when you do.


REMEMBER
If it is not possible to complete the questionnaire in one sitting you can return at a later date and your computer will remember where you were, but not what your previous answers were. It is best to complete a page and send it before ending your session. The entire questionnaire should be completed using only one computer.

COOKIES must be turned on and not deleted until the questionnaire is complete. If cookies get turned off automatically or accidentally, you will be directed to SAMPLE pages, and not be able to complete the questionnaire. Seeing your name at the top of each questionnaire page indicates that cookies are working and that page of your answers will be forwarded to us.

If your name is not at the top of each questionnaire page, please use the link at the top of this page for instructions or click on the help link at the top of any questionnaire page. When on the help page, drop down to the red section which discusses what to do when your name is missing. Please make sure to check at the top of each new questionnaire page for your name. If it is there, you can proceed.

Also, because we use cookies to store your answers, you should not bookmark any questionnaire pages. This may result in your not being able to resume the questionnaire correctly if you return at a later date.

For those using the latest version of Mozilla Firefox. Firefox may block some scripts which are needed for the questionnaire to be functional. At the bottom of your screen you will see an 'options' box. By clicking on this and choosing 'Allow DigitalNaturopath.com', our questionnaire will function normally. You will encounter this same screen with other sites also, and now you know what to do if you didn't already.
